Proposed resolution accepting the Request for Proposal response from OCHS Inc on behalf of Securian Financial Group, Inc, for Request for Proposal No. 2025-26-030 for Life Insurance and AD&D Coverage, and award the Life Insurance and AD&D Coverage through Minnesota Life Insurance Company for an initial term of three (3) years with an option to renew for an additional two consecutive one (1) year term, in a form acceptable to the City Attorney; and in substantial conformity with the referenced Request for Proposal attached hereto as “Exhibit A”; and to issue a purchase order to Minnesota Life Insurance Company in an amount not to exceed $232,722.51, for the Life Insurance and AD&D Coverage; and authorizing the Mayor or designee to sign any related and customary purchasing documents to effectuate this resolution, and providing for an effective date.
